Behavioral Health Care CoordinatorSupport patients throughout their behavioral health journey by keeping care organized, accessible, and connected. As a Behavioral Health Care Coordinator, you'll serve as an important link between patients, families, Behavioral Health Care Managers, providers, and referral sources while supporting a primarily geriatric population. You'll coordinate referrals and appointments, educate patients and decision-makers about the Behavioral Health Program, and help ensure each individual understands what to expect as they begin and continue receiving care.This role is ideal for an organized, compassionate professional who enjoys helping others, solving problems, and coordinating multiple priorities with limited supervision. You'll schedule initial and follow-up visits, coordinate IDG and telehealth sessions, obtain and maintain required consents, manage the behavioral health care registry, and respond to patient and family needs through clear and timely communication.
