Community Program Manager IIThis position is located at the Garlington Health Center in NE Portland, OR. The schedule for this position is Monday through Friday, 8:30 a.m. to 5:00 p.m.Cascadia's Mission and Vision:Mission: Cascadia Health delivers whole health care – integrated mental health and addiction services, primary care, and housing – to promote hope and support the well-being of the communities we serve.Vision: We envision a community where everyone benefits from whole health care, experiences well-being, and has a self-directed, connected life.Program Description: The Community Program Manager III is responsible for overseeing and maintaining day to day operations of the permanent supportive housing programs. The role of the program manager is to direct QMHP, QMHA, Peer Wellness and administrative staff in providing treatment and housing retention supports for client households participating in our supported housing programs, while supporting a Trauma Informed Care model. With the high prevalence of trauma among individuals receiving mental health services, it is required the Program Manager understand the effects of trauma on health, coping, and other aspects of the lives of those we serve. This position has a leadership role in inter-agency program development and oversight. The Community Program Manager III explores, evaluates and cultivates program and staff development using research-based best practices or evidence-based models of care in specialized supported housing services settings. This position involves participation and implementation of program goals and objectives to promote a strengths-based, client-centered approach to assisting residents in achieving individual goals and ultimately transitioning to a less restrictive setting. The Program Manager III will provide and oversee a wide range of administrative duties connected to clinical operations and grant requirements.Essential ResponsibilitiesThis position description is not intended to be an all-inclusive list of responsibilities, skills, or working conditions associated with the position. Management reserves the right to modify, add or remove duties as necessary.Contribute to the development of annual goals and objectives for the program which are consistent with the strategic goals of Cascadia Behavioral Healthcare, and the department(s) to which the Program Manager is assigned.Assist the Clinical Director and Senior Director in developing and updating policies, procedures and systems to support agency goals and objectives for permanent supportive housing, short-term rent assistance and program clinical services.Determine staffing requirements, recruit, hire and train staff. Establish work plans and priorities for staff members and assess the progress of staff members; take corrective action as needed. Conduct performance reviews for staff members.Oversee daily operations and provide training and guidance to staff to ensure that program goals and contract objectives are met.Serve as a leader for team related to adherence and training for staff on agency policies/procedures. Conduct regular review and updating of program specific procedures and written guidelines, and initiate action and active leadership in developing, revising, or drafting program procedures, guidelines and protocols in collaboration with peer Management Staff, Clinical Director, Sr. Director, and administrative leadership including Quality Management and Human Resources.Lead in communicating and coordinating with partners and Cascadia Housing property management team for programs.Serve as lead for notification and coordination with Quality Management and Human Resources Employee Relations in events which are Critical Incidents, or impact staff negatively.Assist community partners and stakeholders to resolve interagency and systems problems.Assist Cascadia administrative and operations staff in developing policies and procedures to support operations throughout Cascadia's supportive housing/homeless supported housing programs.Monitor the overall effectiveness of the program(s). Assist in the implementation and evaluation of changes that increase the level of service, overall efficiency, care quality, and contract outcome achievement and reduction in costs.Evaluate, recommend and implement methods, systems, procedures and treatment strategies to manage costs. May have some responsibility for developing new sources of revenue.Prepare reports that reflect key data points for the Clinical Director and Senior Director or other stakeholders as required/requested.Maintain research orientation and serve as the main link between program operations and the translation of best practices or evidence-based models of care into service delivery models utilized.Lead and model crisis interventions in crisis situations as appropriate. Follow emergency protocols and procedures and coordinate with the treatment team interventions which support a trauma informed environment for residents and staff. Practice and demonstrate competency in avoidance, preventing escalation to avoid crisis situations or to neutralize crises in progress. Carry primary responsibility for safety protocols (written, and training) for the team.Model therapeutic interventions and counseling with clients as needed or directed and assist staff members by helping to resolve difficult client/program situations.Assure that staff members document and maintain electronic health record (EHR) and ancillary records in a manner that assures compliance with all agency policies, program procedures and local, state, and federal regulations. Maintain accurate and up-to-date documentation associated with billing, assessment, treatment, residents' progress and other activities, as required.Provide oversight to ensure services are being offered consistently and professionally.Provide therapeutic interventions as directed by clients' Service Plan. If clinically appropriate, complete Behavioral Support Plans; conduct behavioral health assessments and perform case management duties from time to time.Coordinate treatment and continuity of care within the team, as well as with community stakeholders, guardians, representatives from the legal system, hospital care, medical care, and other community based agencies in order to assist with the treatment needs of clients on case load.Respect and honor client rights and responsibilities and demonstrates professional boundaries and ethics.Demonstrate responsibility for safety of clients, staff and property; demonstrate knowledge with fire regulations and evacuation procedures, and oversight of team and facility response.Adhere to mandatory abuse reporting laws and HIPAA requirements.Complete annual employee training requirements on a timely basis as indicated in the licensing requirements for the facility as well as agency requirements. Attend seminars, training and other educational opportunities in order to develop professional skills and abilities.Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings.Perform other duties as assigned.Role Specific ResponsibilitiesResponsible for the daily management of the program including but not limited to the provision of all clinical and medically related services. Structure individual and group clinical services to attain consistency in coordination, provision of therapeutic services, high utilization of clinician and LMP services, and ability of staff to complete all documentation on the same day of service.In collaboration with the larger HOT team, promote a therapeutic, welcoming and safe community environment. Model therapeutic interventions and counseling with program clients; as needed assist team members to resolve difficult client/program situations.Serve as lead for program partnerships.Lead internal partnerships related to Housing Outreach Team's programs.Coordinate effectively with team's administrative support staff to ensure HMIS data entry/updates/exit documentation are handled in a timely and accurate manner.Track grant spending in relation to rental assistance funds. Responsible for ensuring that grant fund spending is within the limits under specific grants in portfolio.Work with Clinical Director to complete and submit contract quarterly and annual reports. May include coordination and data quality assurance across program areas receiving grant funding.Responsible for effective coordination with referrals, including working closely with Cascadia's designated intake/referral coordinator specific to the local Coordinated Entry process and placements for Permanent Supportive Housing project units.Lead for Critical Incident process for team. Ensure crisis event documentation is done in a timely and appropriate manner, including same shift documentation in EMR and crisis alerts.Establish annual operating budget, plan for revenue, and establish strategic goals for successful operation. Revise goals as circumstances dictate. Close oversight of program spending, revenue and appropriate adherence to agency protocols and policies. Responsible for appropriate coordination with fiscal and accounts payable staff within Cascadia.Responsible for collaborative leadership, agenda planning, facilitation of team clinical meetings, including staff meetings, staff trainings, and partnership meetings.Responsible for operating in compliance with city regulations, state licensing standards, county requirements or other applicable guidelines and internal policies and standards.Comply with State of Oregon Outpatient Oars, grant requirements, and Cascadia policies by keeping up to date on information, and provide or arrange for training and updates to the team.QualificationsEducation:A Master's Degree in a behavioral science field from an accredited college or university, or other relevant Master's Degree. Demonstrate the competencies to become credentialed as a Qualified Mental Health Professional (QMHP) as outlined in OAR 309-016-0005.
