Description

American Traveler is seeking a Physical Therapist for a Home Health assignment requiring 1+ year of recent home health experience and HCHB EMR proficiency.

Details

• Home Health setting covering the Anacortes, Oak Harbor, and Coupeville territory
• Day shift schedule, 8am-5pm, Monday-Friday, 5x8 hour shifts
• No weekend or on-call requirements
• Requires in-office visits at least once every one to two weeks for team meetings, supplies, and admission packets
• Caseload of approximately 5-7 patients per day with 6 visits daily and a minimum of 30 productivity points weekly
• Patient population primarily geriatric
• Territory extends up to a 60-mile radius from the main office and may require ferry travel
• Uses HCHB EMR system
• iPad provided for documentation; personal cell phone not required
• Mileage reimbursed at the IRS standard rate

Requirements

• Minimum 1 year of recent Home Health experience required
• BLS certification required
• OASIS experience required
• HCHB EMR experience required
• License required if applicable
• Experience with orthopedic, neurologic, and post-operative patient populations preferred
• Skilled in home exercise program development and patient/family education
• Must be flexible with territory assignments
• Timely documentation and communication required

Additional Information

• Responsibilities include home-based patient assessment, treatment planning, and rehabilitation for geriatric patients
• Scrubs required in any color; no jeans permitted
• Local candidates accepted at the travel rate
• First-time travelers will not be considered for this position
