Community Support Services Specialist, Older Adults

Principal Responsibilities: This is an on-site, full-time role Job Responsibilities:
Agency Information: The Marion & Aaron Gural Jewish Community Center, a non-profit, non-sectarian community agency with four locations in the Five Towns is looking for a social worker to work with older adults.
- Conduct intakes for our older adult programs and other JCC programs and services.
- Perform outreach to synagogues, local organizations, physician offices, health care agencies, government agencies and community members.
- Provide external referrals as appropriate.
- Perform crisis intervention, short-term counseling and connect to concrete services.
- Develop and maintain a resource guide for community referrals
- Coordinate all aspects of assigned older adult program activities including transportation if needed
- Ensure implementation of the agency's policies and practice
- Provide psycho-social client intakes and Person-Centered Care Plans
- Communication with clients, caretakers and families
- Provide an environment of dignity and respect for all clients
- MSW or LMSW preferred with two to three years of experience working with Older Adults
- Must be very familiar with Jewish practice and observance
- Ability to work with diverse teams and clients.
- Program development and outreach
- Strong time-management and organizational skills.
- Ability to multi task and problem solve
- Excellent verbal and written communication skills
- Grant writing experience a plus
