YOU have an exciting opportunity to join our team and work at our exceptional Skilled Nursing Facility! We are currently expanding our staff and are looking for a compassionate and enthusiastic Social Services Assistant!

· Responsibilities

Essential Functions:

• Develop a comprehensive social history assessment, a psycho-social assessment, discharge plan, and complete the MDS and Care Plan for new admits.

• Orient and assist new residents in adjusting to the facility.

• Complete MDS forms in a timely fashion

• Assist residents and families with adjustment to changes, including room changes.

• Assist families with the engraving and/or marking of residents’ personal items.

• Review residents’ rights with residents and staff and attend to various personal needs of the residents.

• Assist in the development of a volunteer program and assist in facilitating family group meetings.

• Document progress notes that relate to each resident’s care plan when necessary and within policy timeframe

• Comply with all Company policies related to safety and infection control procedures including the proper use of mechanical lifts, gait belts and personal protective back supports

• Understand, comply with and promote all rules regarding resident rights, promote positive relationships with residents, visitors and regulators, to include presenting a professional appearance

• Other duties as assigned or needed

Skills

• Proven work experience in long term care

• Working knowledge of social theories and practices

• Social perceptiveness and empathy

• Ability to build and maintain professional helping relationships

• Ability to relate and communicate with diverse population and groups

• Resilience along with ability to assess situations

• Willing to submit to background checks

• Psychology or sociology experience preferred
