Acute Care Manager - Care Coordination

Shift Type: Days - Patient Ratio: 1:25

Requirements:

• California state license required; pending licenses accepted for submission but must be active by start date
• Minimum 2 years of experience as an acute medical Care Manager with a focus on care coordination and discharge planning
• Experience with HMOs/IPAs and utilization/medical-necessity assessment preferred
• Epic charting experience required

Roles & Responsibilities:

• Coordinate patient care and discharge planning for adults and geriatric populations.
• Assess patients' needs and develop comprehensive plans of care.
• Manage a caseload of up to 25 patients daily, ensuring compliance with admission criteria and regulatory requirements.
• Collaborate with interdisciplinary teams to enhance patient outcomes and streamline care transitions.
• Conduct utilization reviews to ensure appropriateness of care based on CMS/DRG guidelines.
• Ensure HIPAA compliance in all patient interactions and documentation.

Compliance / Notes:

• Orientation: 40 hours
• Weekend requirement: Typically one full weekend every 4 weeks, subject to department needs.
• Must commit to working 3 of the 5 listed holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).
• Scrub/dress code: Business attire or scrubs.
