Title Physician Assistant Company Oak Street Health Role Description The purpose of a Physician Assistant at Oak Street Health is to provide effective and equitable value‑based primary care to adults on Medicare to keep them happy, healthy, and out of the hospital. Physician Assistants see patients independently and collaborate with physicians, depending on the patient’s specific situation. They provide longitudinal care, build meaningful patient relationships, and focus on care coordination and population health within outpatient practices on collaborative care teams. Core Responsibilities Provision of exceptional primary care. Conduct office visits for routine and acute issues. Administer annual wellness visits and health risk assessments, requiring a holistic view of health and thoughtful, accurate documentation. Coordinate care with other providers, specialists, testing facilities, and agencies. Lead population health activities—e.g., ensuring eligible females receive evidence‑based breast cancer screening every two years. Assist the care team with phone triage and outreach. Educate patients on their health conditions, care plans, and treatments. Participate in Oak Street Health promotional activities. Conduct home visits as needed. Perform additional duties as assigned. Required Qualifications 3 years primary care experience (preferred). Active, non‑probationary state Physician Assistant license. Active DEA license. U.S. work authorization. Must embody the values and character of Oak Street Health. Preferred Qualifications Experience in internal medicine, geriatrics, or family medicine. Understanding of HCC documentation, ICD‑10 coding, and health risk assessments. Passion for teamwork and willingness to collaborate cross‑functionally. Pay Range The typical pay range for this role is $95,738.00 – $206,206.00.
