LTSS Care Manager: Integrated Care & Waiver Specialist

Molina Talent Acquisition

Molina Healthcare in Wisconsin seeks a Care Manager to support LTSS care coordination and oversee member progress across the continuum. You will complete comprehensive assessments, develop and monitor care plans, and collaborate with the multidisciplinary team to coordinate services including behavioral health and LTSS.

The role requires 2+ years in health care, licensure if required, a valid driver’s license, and strong communication skills.
