Aftercare Case Manager - Help Families Thrive
Mainstream Nonprofit Solutions, Inc.
TFI Family Services in Kansas is hiring a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, guardianship, or aging out of care. You will focus on risk management, provide crisis intervention, maintain documentation, coordinate community services, and meet with families in person to support reunification. Ideal candidates have a bachelor’s degree in human services, a valid driver’s license, and a willingness to learn #J-18808-Ljbffr Mainstream Nonprofit Solutions, Inc.
