Fieldston Lodge Center is looking for Professional, Caring, Energetic and Eager LPNs to be apart of our team.

Fieldston Lodge is a true home away from home, situated in beautiful Riverdale, nestled in a scenic park overlooking the Hudson River and the Palisades. Fieldston Lodge provides 24-hour care along with many other services to ensure you or your loved one will receive the highest quality of care. Fieldston Lodge Care Center is a 200 bed skilled nursing facility providing both long and short term care.

We are currently seeking Full Time/ Part Time and Per Diem LPNs for all shifts.

SIGN ON BONUS of $2000.00 / $1000 after 3 months and another $1000 after 6 months

Job Description:

• Taking and monitoring patient vital signs 
• Assisting patients with eating, dressing, and bathing 
• Administering medication 
• Communicating changes in patient behavior or health status to Nurses, Physicians, and family members 
• Maintaining accurate patient records 
• Collecting specimen samples when needed 
• Taking an active part in patient care planning 
• Overseeing Certified Nursing Assistants and training 
• Preparing patients for transfers or discharge 
• Assisting in medical procedures 
• Managing emergency medical situations 
• Accurately document observations, interventions, and evaluations pertaining to resident to electronic medical records.
• Order prescribed medications, supplies, and equipment as needed.
• Follow a designated care plan in accordance with residents needs.
• Perform assigned duties including wound care, treatments, and procedures
• Maintain confidentiality and rights of residents, family members, employees, and facility information in compliance with HIPAA policies.

Requirements:

· Licensed in New York State

· CPR certified

· Good standing with State Registry

· Must be a willing to work together as a team to ensure the goals at hand are achieved.

Qualifications:

· New Grads welcome

· Ability to work every other weekend

· Excellent customer service skills

·

· Committed to working assigned schedules with the ability to be punctual and have a good attendance record
