Summary This is for a Clinical Pharmacist with the Aleda E. Lutz Veteran Affairs Medical Center. The clinical pharmacist is a licensed professional with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics. The incumbent is responsible for analyzing real and potential drug-related medical problems and implementing corrective action to ensure that patients receive optimal drug therapy. Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. United States Citizenship: Non‑citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA policy. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. § 7402(d) and § 7407(d). Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. The pharmacist must maintain current registration if this is a requirement for maintaining a full‑current‑and‑unrestricted license. A pharmacist who has or has ever had any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, PartII, Chapter3, sectionB, paragraph16. Individuals who have had multiple licenses revoked for professional misconduct or who surrendered a license after written notice of potential termination for misconduct are not eligible unless the license is fully restored (38 USC § 7402(j)). Effective November 30 1999, this is a requirement for employment. Non‑licensed pharmacists who otherwise meet eligibility may receive a temporary appointment as a Graduate Pharmacist under 38 U.S.C. § 7405(c)(2)(B). For grade levels above GS‑11, the candidate must be licensed. In all cases, pharmacists must actively pursue meeting state prerequisites for licensure starting from the date of appointment. Failure to become licensed within 2years from appointment will result in removal from the GS‑0660 Pharmacist series and possible termination of employment. Employees grandfathered into the GS‑660 series may be promoted up to full performance level but not beyond or into managerial positions. Grade determinations: GS‑11 requires experience, education, and licensure; GS‑12 requires one year of equivalent experience or completion of an ACPE‑accredited Pharm.D. program. The employee must demonstrate the knowledge and skills necessary to complete duties, including professional pharmacy practice, communication with patients and health‑care staff, knowledge of laws and regulations, and monitoring and assessment of drug therapies related to laboratory and diagnostic parameters. The full performance level of this vacancy is12, with actual grade range11–12. Duties Duties include but are not limited to: providing appropriate selection of drug therapy based upon pharmaceutical principles, monitoring for efficacy, side effects, and clinical outcome, and advising prescribers as appropriate. Providing patient‑specific therapeutic drug monitoring and communicating relevant findings to other health‑care providers orally and in writing. Monitoring therapeutic appropriateness, duplication, route, compliance, drug‑drug, drug‑food, drug‑laboratory, and drug‑disease interactions, and relevant laboratory data to anticipate side effects and adverse effects. Performing continuous evaluation of prescribed medications to assure optimal drug therapy. Providing medication counseling to patients. Reviewing all medication orders for appropriateness—including drug selection, dosage, route of administration, and amount—prior to dispensing. Ensuring all medication orders are entered into the electronic patient medical record and filled in a timely fashion. Reviewing the patient’s allergy history, drug‑drug, drug‑nutrient, drug‑laboratory, and drug‑disease interactions for clinical relevance. Providing education to health‑care providers regarding medication use. Maintaining basic knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics in the area of practice. Other duties as assigned.
