Home Care Field Registered Nurse– Columbia County, NY | $85K Location: Columbia County, New York Company: At Home Care Partners Salary: $85,000 per year Position: Full-Time Home Care Field RN Company Slogan: "You're Not A Patient You're Family" Home Care Field Registered Nurse (RN) At Home Care Partners, a Licensed Home Care Agency, is seeking a compassionate and dependable Home Care Field Registered Nurse (RN) to provide high-quality nursing care to clients throughout Columbia County, New York . This opportunity is ideal for an RN who enjoys working independently, building relationships with patients and families, and providing personalized care in home and community-based settings. As a Home Care Field RN , you will travel throughout Columbia County to complete nursing assessments, oversee patient care, conduct aide supervisions, and develop individualized Plans of Care. Key Responsibilities Travel to client homes and community-based settings throughout Columbia County Conduct comprehensive in-home and community-based nursing assessments Perform head-to-toe evaluations, including medication and diagnosis reviews Complete aide supervisions and provide clinical guidance to caregivers Develop, review, and update individualized Plans of Care (POC) Orient aides to each client's Plan of Care and specific care needs Monitor client conditions and report changes or concerns as appropriate Communicate effectively with patients, families, caregivers, aides, and office staff Complete accurate and timely clinical documentation Help ensure services are delivered safely and according to each client's individual needs Maintain compliance with applicable healthcare regulations and company policies Registered Nurse Requirements Current and active New York State Registered Nurse (RN) license Ability and willingness to travel throughout Columbia County and assigned service areas Medical Professional Liability insurance Strong nursing assessment and clinical documentation skills Comfortable using Gmail and Microsoft Word/Excel Excellent communication and interpersonal skills Ability to work independently in a field-based environment Professional, reliable, organized, and compassionate Must meet all company hiring and onboarding requirements Preferred Qualifications Previous home care, home health, outpatient nursing, or community health experience Familiarity with Medicaid, Medicare, or MLTC plans Experience conducting patient assessments and aide supervisions Experience developing, reviewing, or updating Plans of Care Strong clinical assessment and care coordination skills Bilingual in Spanish and English Comfortable working with patients from diverse backgrounds and with varying healthcare needs Compensation & Benefits Salary: $85,000 per year At Home Care Partners offers: Competitive compensation Weekly direct deposit Health insurance Dental insurance Vision insurance 401(k) plan Transit benefits and dependent care accounts Verizon Wireless and mobile internet discounts Company vehicle and parking access, where applicable Supportive team culture Opportunities for advancement Why Join At Home Care Partners?
