Hospice Admission Nurse - Sign-On Bonus
Community Healthcare of Texas
Community Healthcare of Texas is seeking an Admission Nurse to meet with patients and families, explain hospice services, and coordinate admission. The role includes thorough admission assessments, planning initial nursing visits, coordinating DME/RX needs, and collaborating with the Interdisciplinary Team. Candidates must be a Texas-licensed RN with at least 1 year of nursing experience; hospice case management is preferred.
