RN Case Manager - Discharge & Care Coordination (PRN)
Houston Methodist
Houston Methodist is seeking a licensed RN for the PRN Case Manager role on designated unit(s). You will plan case management for a targeted patient population, collaborate with physicians and the interprofessional team, and ensure admissions and ongoing stays are medically necessary while communicating with payors for reimbursement. You will share accountability with social work for discharge planning and contribute to performance improvement initiatives, education, and patient satisfaction #J-18808-Ljbffr Houston Methodist
