Home Health Registered Nurse

Position Overview

We are seeking an experienced Registered Nurse to provide skilled home health services with a primary focus on OASIS assessments and coordination of care. This role serves patients across Monongalia, Marion, and Harrison Counties and requires strong clinical judgment, accurate documentation, and collaboration with the interdisciplinary care team to ensure high-quality patient outcomes.

Schedule & Work Details

• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Weekly Hour Guarantee: 32 hours
• Productivity Expectation: 25 to 30 points per week
• Coverage Area: Monongalia, Marion, and Harrison Counties
• On-Call/Weekend Requirement: At least one weekend per month, one Friday, and four additional weekdays as part of the rotation
• Mileage Reimbursement: Yes, reimbursed between patient visits
• Pay Rate: $51 to $53 per hour

Key Responsibilities

• Complete OASIS visits including start of care, recertifications, and other required assessments
• Coordinate patient care following OASIS completion, including ongoing skilled nursing visits
• Perform comprehensive nursing assessments and update individualized plans of care
• Communicate patient status changes and care needs with physicians and the interdisciplinary care team
• Accurately document all visits, assessments, and care provided in Homecare HomeBase (HCHB)
• Ensure compliance with agency policies, infection control standards, and state, federal, and CMS regulations

Required Qualifications

• Active Registered Nurse license
• Proficiency with Homecare HomeBase (HCHB)
• Demonstrated experience completing OASIS assessments
• Ability to work independently in a field-based role
• Strong assessment, documentation, and organizational skills
