As an MDS Coordinator, you will play a critical role in ensuring the accurate and timely completion of the Resident Assessment Instrument (RAI) process for each resident in our facility, as required by state and federal regulations. About the Role

As an MDS Coordinator, you will be responsible for:

• Completing the RAI process for each resident, including transmission and verification of receipt by the state
• Collaborating with the interdisciplinary team to ensure accurate and comprehensive assessments
• Maintaining accurate and up-to-date records of resident assessments and care plans
• Participating in quality improvement initiatives to improve resident outcomes and satisfaction

The Ideal Candidate

• Has a valid Texas Nurse License and valid CPR Certification
• Has a minimum of three years of experience in a supervisory capacity in a hospital, skilled nursing facility, or other related healthcare program
• Possesses knowledge of long-term care operational standards as set forth in the Federal Register, Conditions of Participation, and state regulations
