BLS certification required. Massachusetts nursing license required with Primary Source Verification within 30 days of start date. Fit Test (N95) within 365 days to start date and updated annually. Respirator Medical Clearance (RMC) Attestation required if fit test is required. MGB Medication Exam with 80% passing within 365 days for RN. Documentation of MMR vaccination or immunity required. TB Test documentation within 3 months of start date and updated if >6 month break. Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break. Criminal Background Check within 45 days prior to start date and updated if >6 month break. Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break. MGB Resume including past 7 years of work experience required for submission. Two references within 4 years of submission, minimum 1 supervisor or above in skillset. MGB Skills Checklist must be completed within 1 year of submission. Confirmation of Assignment/POI Form required for all initial assignments and extensions. Education Verification through background check company required. SSN Trace must be run with Criminal check. National Sex Offender Check ran within 30 days of start and updated annually. Exclusion Checks ran within 30 days of start and annually. License/Certificate Verification required for submission and onboarding. No MA Conditional Licenses accepted. Schedule Information

MGB IRP Resume and Evaluation optional for submission. Hepatitis B vaccination strongly preferred but optional. Tdap vaccine preferred within 10 years and updated upon expiration. Positive PPD requires negative chest X-ray documentation. MGB Name Change Documentation optional if legal name differs from credential documentation. MGB TB Symptom Survey required if Positive PPD, updated annually. Candidate Submission Coversheet required for initial assignment and new facility submissions.
