About This Opportunity
Welltel is launching an exciting new behavioral health program to serve our diverse patient population. We're seeking a pioneering Licensed Master Social Worker who's ready to build something meaningful from the ground up. What You'll Be Doing
Provide therapeutic services via telehealth to patients at our FQHC locations
Collaborate directly with our leadership to develop program protocols, screening processes, and workflows
Conduct initial patient assessments and ongoing therapy sessions
Help establish best practices for behavioral health integration within our primary care model
Work with our team to identify and engage patients who would benefit from behavioral health support
The Ideal Candidate
Must Have:
Licensed Master Social Worker (LMSW) in New York State
Experience providing therapy and behavioral health services
Comfort and proficiency with telehealth platforms
Understanding of FQHC patient populations and their unique needs
