The Incident Management Coordinator (IMC) supports the hospital’s Incident Management process. The IMC helps ensure that unusual incidents, allegations of abuse, neglect, and exploitation (ANE), crisis situations, and health and safety concerns are addressed promptly, thoroughly, and in alignment with state hospital requirements. The IMC plays a central role in protecting patient rights and maintaining a safe hospital environment by reviewing investigations, monitoring remedial action decisions, and providing data analysis that informs performance improvement. The IMC serves as a central point of communication between departments such as Quality Management, Rights Protection, Risk Management, and hospital leadership. The IMC ensures immediate protective actions are taken when needed, follows each incident through its full process, and tracks the progress of prescribed interventions. The IMC also reviews and analyzes incident data to identify patterns and trends to help reduce the likelihood of future incidents. In addition, the IMC assists with developing and updating hospital policies related to incident management, collaborates with external investigators and internal staff, and helps ensure reporting requirements are met. This position works under general supervision with some latitude for independent judgment. Performs other duties as assigned. Other duties as assigned include actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location Essential Job Functions (EJFs): Attends work on a regular basis and may be asked to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. Provides technical assistance regarding entering data into the assigned data management system to those submitting initial and follow-up incident reports regarding required content, level of detail, individuals involved; gathers incident information, investigates and tracks incidents until they are fully reported and resolved. Ensures that protective actions are carried out and reevaluated as new information is obtained, keeps facility leadership and central office informed of new developments, and documents every stage of the incident management process from start to conclusion. 25% Documents the preliminary review of reportable incidents in assigned data management systems, including abuse, neglect and exploitation (ANE) cases, and records team recommendations in required databases and in various formats, and contributes to the development of plans of correction for internal and external reviews, surveys, and investigations that may be conducted by external stakeholders such as Joint Commission (JC), Centers for Medicaid and Medicare(CMS), law enforcement, guardians and any other external stakeholders. 25% Participates with the development and revisions of the hospital’s incident management policies to ensure they meet hospital needs and remain in compliance with both state hospital and external requirements to ensure alignment with JC, CMS, and state and federal laws and regulations. Policies are updated as needed to respond to performance concerns, emerging trends, and regulatory changes. 25% Reviews and analyzes data related to incidents to ensure accuracy and integrity. The IMC identifies trends and patterns that may contribute to unusual incidents and tracks remedial/corrective action plans to reduce the risk of recurrence. Generates the root cause analyses of incident contributing factors, sentinel event reviews (death of an individual in care), and management-Initiated investigational reviews as assigned. 20% Performs other duties as assigned. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location. 5% Knowledge, Skills and Abilities (KSAs): Knowledge of investigative principles, techniques, and procedures; of the laws governing the activities regulated by the agency; and general knowledge of criminal procedures, practices, and rules of evidence. Skill in interviewing victims, witnesses and potential individuals causing harm, injury and or damage to persons or property. Ability to gather, assemble, correlate, and analyze facts; to devise solutions to problems; to develop and evaluate policies and procedures; to prepare reports; to communicate effectively. Able to produce written documents and data analysis reports using Microsoft Office programs. Ability to accurately and clearly interpret/develop pertinent policies/procedures and technical reports. Ability to establish and maintain effective working relationships with staff, individuals, parents, members of the community-at-large, outside state and local investigation agencies or others in work context. Ability to organize, coordinate and delegate routine and complex tasks and supervise the work of others. Ability to develop monitoring systems, collect and analyze information, and make cost effective recommendations. Ability to make effective decisions during unusual incidents/crises. Ability to analyze results and document recommended actions as well as ability to train other staff on investigative techniques. Ability to demonstrate commitment to facility mission and workplace/service values. Registrations, Licensure Requirements or Certifications: Preferred but not required: Licensed to practice as a licensed vocational nurse (LVN) or registered nurse (RN) in the State of Texas (or state that recognizes reciprocity through the Nurse Licensure Compact) prior to hire. OR Licensed Master Social Worker, or Licensed Clinical Social Worker. Initial Screening Criteria: Bachelor's degree from an accredited college or university; major in social work, sociology, psychology, or nursing. OR Four (4) years in one or more of the following fields: Social Work, Sociology, Psychology or nursing. Three (3) years’ experience working in a psychiatric healthcare setting preferred Additional Information: Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aid, Medication Aide and Employee Misconduct, List of Excluded Individuals/Entities (LEIE). Males between the ages of 18-25 must be registered with the Selective Service. All applicants must be at least 18 years of age to be considered for employment at a state-operated facility. All these diseases are vaccine preventable. As a result, state hospital policy requires employees be vaccinated according to their level of contact with individuals.
