Evaluate, treat, document, and help patients/residents improve function, independence, safety, communication, mobility, and quality of life. Collaborate with rehab leadership, nursing, facility teams, families, and care partners. Step into the assignment with professionalism, flexibility, and a patient‑centered approach. What We’re Looking For Active or eligible MO license for Physical Therapist / PT. Prior SNF, LTC, assisted living, or post‑acute experience preferred / New Grad acceptable. Travel experience preferred but not required for the right clinician. Strong documentation, communication, and team collaboration skills.
