Hospice Admissions RN Compassionate Care Advocate
Chapters Health System
Chapters Health System in Florida seeks an Admission RN to assess patients in their residence or hospital and initiate care within our hospice and care services. You will help transition to Care Navigation and represent the organization.
The role requires an active RN license in the state, BLS, 1 year RN experience (hospice or hospital preferred), and experience with EMR/EHR. Strong communication, teamwork, and the ability to manage multiple priorities are essential.
