Registered Nurse (RN) - Case Management

Case Manager

Schedule: 5x8-hour days

Requirements:

2 years of experience preferred
BLS certification required
Experience with medical necessity review, discharge planning, and utilization review
Familiarity with the Epic charting system

Roles & Responsibilities:

Review medical necessity and conduct utilization reviews
Plan for patient discharges and document screenings and discharge plans
Communicate changes in the plan of care to relevant stakeholders
Coordinate community referrals and anticipate patient needs
Conduct concurrent and continued stay reviews, including understanding DRG and benefits eligibility

Compliance / Notes:

State or Compact license must be in hand at the time of submittal
All requests for time off must be listed at the time of submittal
Previous staff must wait 1 year before returning as a traveler
Core items and lab requirements must be completed within 48 hours of contract signing
Orientation modules are to be completed onsite and are non-billable
