Description

• Assess and evaluate patients within both inpatient and outpatient settings, crafting customized treatment plans.
• Implement therapeutic interventions aimed at enhancing mobility, strength, balance, and overall function.
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation experts to achieve optimal patient outcomes.
• Guide patients and their families through therapeutic goals, discharge processes, and recommended home exercises.
• Maintain clear documentation of evaluations, treatments, and patient progress according to facility and regulatory standards.
• Engage in care team rounds and interdisciplinary discussions to ensure cohesive care strategies.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent accredited qualification.

Certifications

• Active state licensure as a Physical Therapist.
• CPR certification preferred.
• Additional specialized certifications (neurological, geriatric) are a bonus but not mandatory.

Skills

• Relevant prior experience is highly desirable.
