Position Overview

We are seeking a dedicated and passionate Nurse Practitioner to join our team at Vital Key Health Clinic in Ithaca, New York. In this full-time, onsite role, you will be responsible for providing comprehensive healthcare services to our diverse patient population, emphasizing holistic care and the promotion of health equity. This position offers a competitive salary range of USD 150,000 to 160,000 annually, commensurate with experience and qualifications.

Work Environment

Your practice will be primarily housed within a state-of-the-art hospital facility that integrates advanced medical technologies with a patient-first approach. Our team is composed of highly skilled healthcare professionals, allowing for a supportive and innovative work environment. Operating hours are from 9:00 AM to 6:00 PM, with occasional evening clinics until 8:00 PM, enabling flexibility in patient care.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients and their families, utilizing diagnostic tools and medical knowledge to evaluate their health status.
• Diagnose and manage acute and chronic medical conditions, offering both preventive care and health promotion strategies tailored to individual patient needs.
• Collaborate with local organizations and stakeholders to establish and implement impactful health education programs that address community health concerns.
• Advocate for patients’ rights and equitable access to healthcare services, ensuring culturally sensitive care that resonates with the varied backgrounds of our patient population.
• Coordinate referrals and provide follow-up care, ensuring cohesive treatment plans that enhance patient health outcomes.
• Maintain thorough and accurate medical records to comply with industry standards and regulatory requirements, ensuring continuity of care and effective patient communication.
• Engage in community outreach initiatives, including participating in health fairs and educational events to raise awareness and provide essential health screenings.

Qualifications
