Job Description:


- Join a dynamic team focused on delivering exceptional rehabilitative care!
- Utilize evidence-based practice to enhance clinical interventions.
- Provide skilled and medically necessary treatments for patients with physical functioning disorders, supervised by a Physical Therapist.
- Administer a variety of rehabilitation procedures, including:
- Manual techniques
- Ambulation
- Therapeutic exercises
- Modalities
- Supportive and assistive devices
- Assess patient progress and communicate with supervising Physical Therapist to tailor ongoing treatments.
- Report treatment effectiveness to ensure optimal patient outcomes.
- Provide consultation and counseling to patients, families, caregivers, and service providers.
- Collaborate effectively with supervisors and health team members regarding patient care.
- Contribute to discharge summaries and maintain comprehensive patient records.
- Document all required information accurately in the electronic documentation system.
- Participate in essential meetings such as patient care conferences and utilization reviews.
- Engage in required training and professional development opportunities.
- Work with supervising Physical Therapist to secure necessary DME or supplies.
- Mentor and support students under the guidance of the Physical Therapist.
- Adhere to APTA and State Required practice acts, code of conduct, and ethics in your practice.
- Ensure active State Licensure as required.

Requirements:


- 1+ years of experience as a Physical Therapist Assistant (REQUIRED).
- LTC or Rehab experience (REQUIRED).
- Active PTA license at the time of submission (REQUIRED).
- New graduates considered if they have Level II experience.
- Must provide a resume and skills checklist within the last year.
- 0-1 reference at the time of submission.
