Homecare Hub also partners with Health Systems to discharge patients from hospitals who are unable be safely cared for in their existing home environment. Homecare Hub has a strong footprint across Wisconsin, and is arriving in Green Bay in partnership with Emplify's Bellin Health System. The goal is to discharge complex patients from their health system into Homecare Hub's network of personalized, small, residential assisted living homes. Here is an examples of the success between Homecare Hub and Emplify's Gundersen Health system in La Crosse: Role Description This is a hybrid role for a Nurse Navigator at Homecare Hub. This individual will perform assessments of patients in hospital and help with placement of patients into small homes, and as well as oversee a cohort of patients living in small residential homes to assure their clinical healthcare and non-clinical needs are met. This hybrid role is located in Green Bay, Wisconsin and surrounding areas. There may be occasional travel across the state. Most in person work will be local, and there will be a component of work from home as well. Qualifications The individual can be either a nurse, physiotherapist, or occupational therapist. There may be opportunities for a related social worker role. Experience in the healthcare or caregiving industry. Ideally some inpatient hospital experience. Excellent bedside patient communication, interpersonal, and leadership skills. Bachelor's or Master's degree in Nursing (or OT, PT, or Social work potentially) Knowledge of Medicaid and Medicare and various plans Knowledge of Support and Social Services in the Green Bay region is helpful Technology skills are an asset
