CASE MANAGEMENT MANAGER

Overview The Case Management Manager assists the Director with the development of the case management program and its implementation at the hospitals. This includes oversight of all case management staff, development and implementation of case management policies and procedures, and coordination of care with other healthcare providers and community resources. The Manager of Case Management plays a key role in ensuring that the program meets its goals and objectives, and that it is compliant with all applicable regulations. Actively and consistently contributes to department operations and communications. Responsibilities Oversee case management staff and operations. Develop and implement case management policies and procedures. Coordinate care with other healthcare providers and community resources. Ensure program compliance with applicable regulations and achievement of goals and objectives. Contribute to department operations and communications. Qualifications State of California Registered Nurse license. Certified Case Manager (CMC or CCM) or Accredited Case Manager (ACM). Minimum of five (5) years\' experience in Clinical Nursing. Minimum of one (1) year experience in case management, preferably in a hospital setting. Critical thinking, service excellence and strong interpersonal communication skills; ability to read and comprehend written instructions; strong organizational skills; ability to follow verbal instructions; and PC skills. Ability to learn, synthesize, make critical judgments, work independently, place patients and families first, and collaborate with healthcare leaders. Position Pay Range: $99,840.00 - $125,580.00 annual salary. In compliance with the California Pay Transparency Act, El Centro Regional Medical Center (ECRMC) posts the pay range for all jobs. Actual pay will be determined based on relevant experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. This range applies to employees who reside in California; a geographical pay differential may be applicable for remote employees who reside out of state.
