Registered Nurse – Home Health

Schedule: 8:00 AM - 5:30 PM, Monday - Friday

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40

Requirements:

• Home Health experience: Required
• Active BLS certification or willing to obtain: Required
• Valid driver's license and car insurance: Required
• Copy of active nursing license: Required
• OASIS and start of care experience: Strongly preferred
• COVID and FLU vaccination: Required

Roles & Responsibilities:

• Prescribe, coordinate, and deliver patient care through the nursing process while maintaining professional nursing standards.
• Supervise Certified Nursing Assistants (CNA) and coordinate care with other healthcare workers as assigned.
• Conduct comprehensive initial assessments and continually assess patients' physical, psychosocial, environmental, and community resources across their lifespan.
• Initiate and modify Plans of Care as needed based on patient evaluations and establish discharge plans in collaboration with patients and healthcare teams.
• Complete OASIS documentation and ensure compliance with federal, state, and agency regulations.
• Assess and address educational needs of patients, families, and caregivers while implementing appropriate teaching strategies.
• Promote a clean and safe environment of care while ensuring patient privacy and confidentiality.
