Discharge & Care Coordination Social Worker
Catholic Health Service
Catholic Health Services is seeking a Social Worker to coordinate discharge planning for residents, addressing psychosocial needs and acting as a liaison between residents, physicians, and nurses. You will help residents access community resources, conduct psychosocial histories, educate on Resident Rights and advance directives, and participate in Care Plan meetings. Requirements include MSW/BSW or related degree, 1 year supervised healthcare social work, CPR certification, computer/EMR #J-18808-Ljbffr
