Join a dedicated healthcare team in the Midwest region as a Homecare Physical Therapist, where you will develop personalized care plans tailored to each patient's unique needs and goals. This role involves evaluating patients for home health or hospice services, collaborating closely with interdisciplinary teams, and supervising Physical Therapist Assistants (PTAs) to ensure optimal patient outcomes.

Key Responsibilities

• Conduct comprehensive initial assessments within designated timeframes and create individualized, age-appropriate care plans focused on self-management and measurable goals.
• Maintain ongoing communication with physicians regarding patient progress and any emerging issues, ensuring timely updates to care plans.
• Engage patients and caregivers in care and discharge planning, adapting education to their learning preferences and overcoming barriers.
• Manage resources efficiently, including scheduling visits and supplies, while completing all documentation promptly.
• Supervise PTAs and homecare assistants as assigned, providing guidance and documenting supervision activities.
• Collaborate with multidisciplinary teams through meetings and direct communication to coordinate patient care.
• Assist patients in obtaining necessary equipment and initiate discharge planning from admission.
• Deliver treatments following physician orders and agency protocols, adhering to infection control and safety standards.

Education

Bachelor's degree in Physical Therapy from an accredited program recognized by the American Physical Therapy Association and the American Medical Association is required.

Experience

At least one year of experience in an acute care hospital setting and one year in a rehabilitation facility or equivalent serving adults is required. Six months of home care experience is preferred. Computer proficiency is a plus.

Certifications and Licenses

• Valid driver's license
• Current state license to practice physical therapy
• American Heart Association Basic Life Support (BLS) certification

Physical and Mental Requirements

• Ability to stand and walk for extended periods and navigate stairs
• Strong communication skills, both oral and written
• Excellent interpersonal and organizational abilities
• Capability to safely operate a vehicle for 2-4 hours daily
• Ability to maneuver around home obstacles and tolerate environmental factors such as temperature changes, smoke, dust, and pets

Working Conditions
