Job Summary

We are seeking a dedicated and experienced Registered Nurse (RN) Team Lead for a 13-week contract assignment at a Skilled Nursing Facility in Montour Falls, New York. This role involves providing comprehensive direct patient care, coordinating nursing activities, and leading a team of healthcare professionals to ensure the highest standards of patient care for residents requiring skilled nursing services.

The RN Team Lead will be responsible for assessing, monitoring, and documenting patient conditions, implementing evidence-based practices, and collaborating with an interdisciplinary team. This position requires strong leadership, critical thinking, and the ability to effectively delegate tasks and prioritize patient needs within a dynamic skilled nursing environment. Flexibility to float to other units and support as an LPN Med Cart nurse, as staffing permits, is an essential component of this role.

Key Responsibilities

• Provide direct patient care, including comprehensive assessment, monitoring, recording, and reporting of changes in patient conditions.
• Demonstrate strong time management, critical thinking, delegation, and prioritization skills in daily operations.
• Advocate for patients' rights and needs, ensuring ethical and professional practice within the scope of nursing.
• Promote and integrate evidence-based practices and best practices into all aspects of patient care.
• Maintain clinical competence specific to the care area and complete all required organizational education within specified timeframes.
• Coordinate patient care effectively with the interdisciplinary team to ensure holistic and continuous care.
• Communicate effectively with patients, families, and the healthcare team, and document all patient information appropriately and thoroughly in the Electronic Medical Record (EMR).
• Participate actively in unit councils and staff meetings to contribute to unit improvements and team collaboration.
• Be prepared to float to other units as staffing needs dictate.
• Be prepared to support as an LPN Med Cart nurse as needed and staffing permits.

Required Qualifications

• Current Registered Nurse (RN) license.
• Demonstrated experience in a Skilled Nursing Facility setting.
• Ability to lift up to 50 lbs.
• Ability to sit or stand for extended periods.
• Ability to physically operate all equipment required for the job.
