Hospice RN & Family Care Coordinator

Gentiva is hiring a Hospice RN / RN Case Manager to lead care at patients' homes, coordinating with doctors, LPNs, CNAs, and social workers. You will perform initial and ongoing assessments, document care, and adapt plans to patient needs while supporting families emotionally and educationally.
