Position Overview

Providence Home Health is seeking a Physical Therapist to serve patients throughout Seaside and the surrounding Oregon Coast communities. This position offers the opportunity to provide individualized, one-on-one care in the home setting while enjoying the flexibility and autonomy that make Home Health one of the most rewarding therapy specialties. Therapists will evaluate patients, develop treatment plans, and help individuals maximize safety, mobility, and independence in their homes.

• Productivity Standard: 30 Points

Requirements

• Active Oregon PT License (Pending License Accepted)
• Minimum 1 Year Skilled Home Health Experience Required
• OASIS Experience Required
• Current BLS Certification
• Reliable Transportation Required
• COVID Vaccination Series Plus Booster Required
