Job Description
Title: Community Health LPN

Schedule: 8:00 AM - 4:30 PM, 8 hour days

Assignment Details:
- Contract Length: 13 weeks

Requirements:
- Current Massachusetts LPN license
- Active American Heart Association BLS or American Red Cross Certification
- One year of acute nursing care experience required; hospital-based experience preferred
- Experience in home health is required
- Certified Home Health experience including proficiency with OASIS is highly preferred
- Current driver's license and insurance are required
- Experience with self-scheduling of caseload preferred

Roles & Responsibilities:
- Provide and document nursing care for patients at home.
- Collaborate with healthcare providers to coordinate care for individuals and families.
- Evaluate and respond to patient and family needs and interventions.
- Function independently within established guidelines and protocols.
- Demonstrate effective time management, communication, and critical thinking skills.
- Maintain confidentiality of patient information and develop clinical records.

Compliance / Notes:
- Must include agency Massachusetts Temporary Nursing Service Agency License with submissions.
- Will not consider any candidate residing within 50 miles of the facility.
