We are seeking a Hospice Nurse Case Manager to be responsible for identifying patient/family needs and for providing skilled nursing assessment, planning, and supportive care and appropriate services in accordance with the attending physician's plan of treatment and orders.

Schedule:

Monday - Friday; 8:30am - 5:00pm

Location:

Alameda and Contra Costa Counties

Hope Hospice follows all CDPH vaccine requirements for healthcare personnel. Essential Duties & Responsibilities include (but are not limited to):

• Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing, and evaluating phases of the nursing process. Maintain a caseload and productivity per agency's protocol.
• Initiate communication with attending physicians, other hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
• Maintain regular communication with Director of Patient Services/designee to review caseload.
• Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions.

Education/Experience:

• One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing.
• Experience in acute care/home health and/or hospice setting preferred.

Certificates, Licenses, Registrations:

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.
• Current California Driver's License and automobile insurance that meets State of California requirements, if driving a car.

Pay Range: $58.90 - $73.65 hourly
