Our organization is in search of a Nurse Practitioner to collaborate closely with a Medical Oncologist at Northern Dutchess Hospital located in Rhinebeck, NY. This role is part of a multi-disciplinary, integrated care model that focuses on specific disease sites. The selected candidate will coordinate, supervise, and deliver advanced nursing care to patients undergoing medical oncology treatment, acting as a Clinical Patient Navigator for this demographic.

Key Responsibilities

• Collaborate with physicians and nursing staff to ensure comprehensive patient care.

• Perform thorough health histories and physical assessments for patients.

• Manage treatment infusions and address supportive care requirements.

• Issue orders in accordance with the established practice agreement.

• Maintain clinical documentation in line with group practice standards.

• Engage in clinical quality initiatives and research as necessary.

• Provide education to patients, families, and caregivers regarding treatment plans.

• Participate in professional development opportunities.

Salary Range

The salary for this position ranges from $120,000 to $150,000, based on experience.
