JOB SUMMARY

The primary responsibility of this position is to serve at-risk/struggling families in the community through the Family Support Program. The Family Support Worker will assist families by engaging families in case management services, linking families to tangible resources through One More Child’s Compassion Center, community resources, and facilitating evidence-based interventions to increase the protective factors, permanency and well‐being of children and families.

ESSENTIAL DUTIES AND FUNCTIONS

• Maintain a caseload of 10-12 families as assigned by the Family Support Supervisor, but not to exceed 15.
• Engage families in services and assist in facilitating evidence-based interventions, to include but not limited to Trust Based Relational Intervention (TBRI)®, Family Check Up® (Everyday Parenting), Motivational Interviewing, and Parent Café®.
• Maintain weekly or bi-weekly contact with all families on active caseload by means of home visits, office visits, phone contact or other correspondence.
• Work collaboratively with parents/caregivers to develop individualized family service plans, based on identified service needs of their families. Assist parents/caregivers with completing outlined tasks and goals related to their family service plans, within the Family Support Program’s scope of services.
• Assist families by linking them to community resources to address identified barriers.
• Assist families with acquiring tangible resources from the Compassion Center.
• Meet with Family Support Supervisor on a regular basis for supervision and the Family Support Program Team to review cases, training, professional development, and program development.
• Complete data entry, maintain contract compliance, and complete required documentation in a timely, accurate, and efficient manner.
• Attend and participate in training required and related to the program’s contract compliance with partnered funder/s.
• Participate in continuing education opportunities including agency in-service training, professional seminars, workshops, conferences, etc.
• Participate in outreach and engagement opportunities related to sharing information about the Family Support Program to increase referrals of families within the program’s service areas.
• Other assignments as needed or requested by the Family Support Supervisor

REQUIRED EXPERIENCE AND QUALIFICATIONS

• High school diploma or GED with minimum two years of experience working with families in the welfare system, at-risk families, or providing case management services

  OR
• Bachelor’s degree in social work, human services, or related field

PREFERRED EXPERIENCE AND QUALIFICATIONS

• Bachelor’s degree in social work, human services, or related field
• Bilingual: Fluent in Spanish and English
• Experience in providing case management services in community settings

• Ability to solve complex, practical problems and effectively deal with complex variables in situations where limited standardization exists.
• Have knowledge of available community resources for children and families in Hillsborough County.
• Ability to create positive relationships with other community partners and agencies.
• Availability to work flexible hours based on needs of the position.
• A valid driver's license.

While performing the duties of this job, the employee is regularly required to sit and talk or hear. The employee frequently is required to use hands to finger, handle, or feel. The employee is occasionally required to walk and reach with hands and arms. Specific vision abilities required by this job include close vision.

This job requires the ability to work in stressful conditions from time to time and remain focused for extended periods of time. Ability to lift up to 30 pounds (boxes, documents, and equipment).

WORK ENVIRONMENT

The work environment describes the conditions that an employee may encounter when performing their job duties, including but not limited to, the essential functions of the position. Reasonable accommodation may be available to enable individuals with disabilities to perform these duties.

Job activities are performed in the community, often visiting clients’ homes and in a typical office environment. Clients’ homes’ physical structure varies in range, with no guarantee of proper accessibility that meets ADA standards. This means, some clients’ homes may require the use of stairs, narrow walking areas and entryways, as well as steps; or lack of certain structures such as grab bars/railings and communication & safety features. This position requires flexibility in scheduling and travel throughout the county, however, no overnight travel or in-person meetings with clients in their homes between the hours of 5PM to 8AM.
