Join a physician-led primary care organization focused on outcomes, not volume, offering a unique opportunity for Primary Care Physicians (MD/DO). This role provides manageable patient panels and strong care team support, allowing you to practice medicine with time, accountability, and purpose. The organization emphasizes value-based Medicare Advantage care and relationship-driven patient engagement.
Practice Info
- Value-based, full-risk Medicare Advantage model
- Patient panel capped at ~450 patients
- Focus on prevention, care coordination, and behavior change
- Dedicated care teams (MA, Care Coordinator, Care Facilitator)
- Emphasis on quality, patient experience, and physician wellness
Responsibilities
- Provide comprehensive primary care to older adult patients
- Maintain full accountability for clinical outcomes and patient experience
- Serve as a health coach, guiding lifestyle and long-term health improvement
- Coordinate specialty and hospital care while staying actively involved
- Lead and collaborate with an interdisciplinary care team
- Supervise and remain accountable for Advanced Practice Providers (APPs)
- Partner with medical and operational leadership to meet quality and cost goals
Compensation
- Salary: $250,000 – $306,714 (based on experience)

Requirements
- MD or DO required
- Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics
- Active or eligible state medical license
- Active DEA
