Direct Support Professional - Per Diem

Job Details Responsibilities Work collaboratively to provide assessment, care, support, and/or rehabilitative services to mentally ill and/or developmentally disabled adults in a residential setting. Assist in the development, delivery, coordination, and monitoring of skill training and recreation planning with residents and staff. Provide supportive direction and hands‑on guidance in life‑skills training to residents. Create an environment for diverse social skills training utilizing in‑house and community resources. Assist residents with planning, purchasing, and preparing meals; support housekeeping, facility maintenance, and laundry. Complete all documentation and record keeping as outlined by unit guidelines and regulatory requirements. Ensure objective documentation; report resident behavior or events in factual terms in progress notes, communication books, and Medicaid billing log. Complete bi‑weekly progress notes and a monthly restorative service note summarizing time frames, events, resident progress, liaisons, and pertinent information. Maintain accurate medical billing information that reflects individual service plans, including service identification, duration, delivery, and outcome. Provide input to behavioral specialists for implementation and evaluation of individual service plans. Deliver effective person‑centered care, recognize resident challenges, and ensure access or referral to psychiatric, medical, and dental needs. Identify changes in resident behavior, mood, and affect; report findings and take necessary actions. Have working knowledge of psychotropic medications and supervise self‑medication of residents per policy. Identify signs of alcohol or drug intoxication and intervene when needed. Ensure accuracy of self‑medication documentation and controlled substance counts. Remain flexible and available for emergency, holiday, relief, and weekend shift coverage as needed. Qualifications High School Diploma/GED required. Bachelor’s degree preferred. Experience with special‑needs or mentally disabled adults preferred. Ability to write legibly required. Maintain a valid NYS driver’s license within agency criteria. Salary Range USD $23.73 - $29.07 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
