Physical Therapist (PT) - Home Health (Olney Region)

Overview Sign on bonus available: $5,000. Relocation bonus available: $2,500 for relocation greater than 50 miles. This position is open to external candidates only. The Home Care Physical Therapist evaluates and develops treatment plans for patients of all ages. The therapist modifies the plan of care as appropriate, serves as a role model in the delivery of professional services, and acts as a clinical resource for staff and students. Hours may vary depending on census and program needs. Qualifications Proof of Auto Insurance – Varies Basic Life Support (BLS) within 30 days – American Heart Association (AHA) Driver's License – Secretary of State (SOS) Licensed Physical Therapist (PT) – Illinois Department of Financial and Professional Regulation (IDFPR) Bachelor’s Degree in Physical Therapy (Required) Responsibilities Documents comprehensive details of each patient visit, providing sufficient information so that staff can determine exactly what care was rendered and the plan for the next contact. Educates and documents discharge planning prior to discharge, including communication with the physician, insurance source, outpatient visits, and community resources. Provides patients with discharge documentation, updates goals promptly, ensures goals are closed when met or changed as patient condition progresses, collaborates in case conferencing with other disciplines (MD, care coordinator, insurers, etc.), prepares written instructions for the assistant, and supervises the assistant in the patient’s home. Administers treatment involving application of physical agents such as heat, cold, light, water, and electricity or traction and massage therapy. Evaluates, fits, and adjusts prosthetic and orthotic devices and recommends modifications to the orthotist/prosthetist. Plans and administers prescribed physical therapy treatment programs to restore function, relieve pain, and prevent disability or loss of bodily function. Performs patient assessment tests, measurements, and evaluations such as range of motion, manual muscle tests, gait and functional analyses, and body part measurements. Records and evaluates findings to aid in establishing or revising treatment programs. Reviews physician or podiatrist referrals and patient medical records to determine if physical therapy is required. Demonstrates comprehension and adherence to regulatory requirements and home‑care policies regarding practice, documentation content, and timelines. Completes functional assessments and documentation per regulations, the comprehensive OASIS assessment of patients, evaluates home‑bound status, and completes a comprehensive physical therapy assessment in the patient’s home to develop a plan of care. Communicates with patients to schedule appointments and manage daily schedule, assesses home safety and recommends equipment to improve safety, educates family or caregivers on home exercise programs, and performs wound care within scope of practice per department policy. Compensation and Benefits The compensation range for this position is $36.74 per hour – $63.19 per hour. This range represents a good‑faith minimum and maximum at the time of posting. Actual compensation will depend on factors including experience, qualifications, location, training, licenses, shifts, and compensation model.
