Experienced Nurse Practitioner in Concierge Medicine

This full-time role is focused on providing exceptional concierge medical services through house calls, enabling us to meet our patients' needs in the comfort of their own homes. Work Environment

This position operates in a unique onsite work setting focused on concierge medicine and house calls. You will have the opportunity to engage directly with patients in their own environments, enhancing their comfort and facilitating meaningful interactions. The role entails a Monday to Friday work schedule from 9 AM to 5 PM, allowing for a balanced professional and personal life.

Key Responsibilities

Your key responsibilities will include:

• Conducting comprehensive patient evaluations through detailed medical histories, thorough physical examinations, and necessary diagnostic tests to establish accurate diagnoses and formulate treatment plans.
• Diagnosing and managing a wide array of medical conditions, including both acute and chronic illnesses, mental health issues, and preventive care needs. You will develop individualized treatment plans rooted in evidence-based practice.
• Prescribing medications and therapies as indicated, clearly communicating treatment options, possible side effects, and expected patient outcomes to facilitate informed decision-making.
• Providing health education and counseling to empower patients with knowledge about their health conditions and guidance on maintaining a healthy lifestyle.
• Collaborating with a multi-disciplinary team, including physicians, specialists, and other healthcare providers, to coordinate care and communicate effectively with patients, ensuring all healthcare needs are addressed.
• Championing preventive care initiatives, such as routine screening, immunizations, and health education, aiming to reduce the incidence of preventable diseases.
• Ensuring precise documentation of all patient interactions in Electronic Health Records (EHR) systems, upholding regulatory compliance under HIPAA and internal policies.

Patient Population / Client Focus

As a Nurse Practitioner at Unihealth MD, you will work with a diverse patient population across various age groups and medical backgrounds. Your role will help bridge gaps in healthcare access, particularly for individuals who may have difficulty visiting traditional medical offices. You will focus on building trust and rapport with patients through personalized care and effective communication.

Qualifications

• A Master’s or Doctoral degree in Nursing obtained from an accredited Nurse Practitioner program.
• A current, unrestricted Nurse Practitioner license in New York State.
• Certification in a relevant Nurse Practitioner specialty, such as Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP).
• Robust clinical judgment with the ability to assess, diagnose, and manage a diverse range of health conditions.
• Proficiency in utilizing Electronic Health Records (EHR) platforms like Epic or Cerner.

Preferred Experience

While a minimum of 2 years of clinical experience as a Nurse Practitioner is preferred, we encourage applications from new graduates who possess strong clinical training and a genuine passion for patient care. Experience in concierge medicine, house call practices, or community health outreach will be highly valued.

Compensation and Benefits
