Acute Care Manager - Care CoordinationShift Type: Days - Patient Ratio: 1:25Requirements:California state license required; pending licenses accepted for submission but must be active by start dateMinimum 2 years of experience as an acute medical Care Manager with a focus on care coordination and discharge planningExperience with HMOs/IPAs and utilization/medical-necessity assessment preferredEpic charting experience requiredRoles & Responsibilities:Coordinate patient care and discharge planning for adults and geriatric populations.Assess patients' needs and develop comprehensive plans of care.Manage a caseload of up to 25 patients daily, ensuring compliance with admission criteria and regulatory requirements.Collaborate with interdisciplinary teams to enhance patient outcomes and streamline care transitions.Conduct utilization reviews to ensure appropriateness of care based on CMS/DRG guidelines.Ensure HIPAA compliance in all patient interactions and documentation.Compliance / Notes:Orientation: 40 hoursWeekend requirement: Typically one full weekend every 4 weeks, subject to department needs.Must commit to working 3 of the 5 listed holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).Scrub/dress code: Business attire or scrubs.
