Home Health Licensed Practical Nurse

Role Overview: A Home Health LPN provides personalized nursing care in patients' homes, supporting recovery and independence. Working under an RN's supervision, you will implement care plans, educate patients and families, document care, and coordinate with the healthcare team. Key Responsibilities Deliver skilled nursing care onsite in patients' homes. Collaborate on care plans with patients, families, and healthcare professionals. Educate on disease management, treatment, and self-care techniques. Maintain accurate documentation of patient visits, interventions, and outcomes. Communicate promptly with team members regarding patient needs and status changes. Qualifications & Skills Current nursing license in the practicing state. Valid driver's license, auto insurance, and reliable transportation. CPR certification. At least two years of clinical experience as an LPN/LVN. Preferred Experience: Home Health or Hospice care experience. Additional Requirements: This role involves patient interaction, TB screening, and driving responsibilities aligned with safety programs. Compensation & Benefits Compensation: Pay ranges from $53,000 to $73,000 annually, based on skills and location.
