Social Services Coordinator

Twin Lakes Extended Care is seeking a Social Services Coordinator to support residents and families throughout the admission, care planning, and discharge process in our skilled nursing facility. Responsibilities: Coordinate and review admissions for appropriate placement Complete social service assessments, BIMS, and PHQ-9 Coordinate discharge planning, including services, equipment, and follow-up care Serve as a resource for residents and families Participate in care plan meetings and collaborate with the interdisciplinary team Maintain advanced directives and accurate documentation in PointClickCare Qualifications: Long-term care or nursing home experience in social services, admissions, or discharge planning Knowledge of Medicaid processes Degree in social services or a related field preferred Strong communication and organizational skills What We Offer: Full-time position Supportive clinical environment Medical, dental, vision, and 401(k) Salary: $18.00-$21.00 per hour #J-18808-Ljbffr
