Home Health RN/ LVN Bilingual PRN

Freedom at Home Dialysis, Expedient Home Health Care and Green Hills Hospice are sister agencies. We are currently looking for a Bilingual RN or LVN to join our Home Health Team. Expedient Home Health provides comprehensive home health services to our coverage area. 

Job Description

Looking for a compassionate and skilled nurse to join our team. You'll play a key role in providing comprehensive care for our patients. 

Your responsibilities will include:

• Conducting initial and ongoing assessments of patients' physical, functional, and psychosocial needs in their place of residence. 

• Developing and implementing personalized care plans to ensure quality and continuity of care.

• Educating patients and their families on disease processes, self-care, and providing wound care and treatments as ordered.

• Ensuring patients meet the criteria for home health

• Providing clinical direction to home health aides and LPNs/LVNs.

• Completing all necessary documentation, including OASIS assessments and progress notes, to track patient progress and ensure a high standard of care.

• Communicating regularly with physicians and the interdisciplinary team about patient needs and changes.

Qualifications

EXPERIENCE AND REQUIRED SKILLS

Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.

• Licensure : Current state license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license.
• Experience : Experience as a RN in a Home Health Setting. 
• Skills : Bilingual Preferred. Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Must have one year home health experience. Must read, write and comprehend English.
