Position OverviewAs an LPN in a long-term care setting, you provide ongoing nursing care to residents, focusing on comfort, function, and decline management. You work within an interdisciplinary care team to support daily living and care plans.The role emphasizes chronic condition management, safety, and coordination with families and clinicians within a residential care environment.ResponsibilitiesDeliver routine nursing care and monitor residents' conditionsAdminister medications and document administrationAssess changes in health status and report to the supervising RNAssist with daily activities and rehabilitation activities as directedCoordinate with physicians, therapists, and social workersDocument care plans, progress, and family communications in the EHRSupport safety, fall prevention, and infection control measuresQualificationsEducation:Diploma or associate degree in nursingLicensure:Active LPN license in the stateCertifications:BLS certification usually requiredExperience or Skills:Experience in long-term care or geriatric settings with care coordination skillsExperienceExperience providing nursing care in LTC facilitiesExperience with documentation in the EHR and care planningWork EnvironmentResidential care setting with a stable team and regular shifts.
