Behavioral Health Rep II - Job Overview
Schedule: Monday-Friday (8AM-4:30PM) | Hybrid
Bring your behavioral health or clinical experience to a role where your compassion, organization, and follow-through help patients receive timely, coordinated care. As a Bilingual Behavioral Health Representative II, you will work closely with on-site Behavioral Health providersespecially Psychiatriststo support patient rooming, referrals, risk assessments, care management, and thoughtful communication between patients and providers.
You will join a collaborative clinic team where every interaction can make a difference in a patient's behavioral health journey. Along with supporting patients and providers, you will assist the Behavioral Health Supervisor, contribute to training and one-on-one shadowing, and help strengthen access to essential community healthcare services.

- Partner with Psychiatrists, Behavioral Health Clinicians, Primary Care Physicians, consultants, hospitals, and community resources.

- Contribute to accurate records, timely information gathering, and coordinated care across the healthcare system.

- Balance direct patient support, care coordination, administrative responsibilities, and team development.
- Make a meaningful contribution to patient access and the broader community healthcare network.
Key Responsibilities
- Provide support to on-site Behavioral Health providers, with a focus on Psychiatrists.
- Take patient vitals and complete patient rooming.
- Manage referrals to providers.
- Screen new and existing patients using risk assessments.
- Take responsibility for patient care management.
- Communicate by phone with providers and patients.
- Assist the Behavioral Health Supervisor with duties as assigned.
- Perform Behavioral Health Representative II duties in addition to all Behavioral Health Representative I duties and provide coverage as needed.
- Manage efforts surrounding BI reporting.
- Obtain collateral patient information from schools and other providers or hospitals as needed.
- Process external referrals in a timely manner.
- Ensure all referrals are completed and relevant documents are included in the medical record, including documentation related to Neurology, speech therapy, cardiology, and other services.
- Review the schedule daily to ensure all pre-visit preparations have been completed before the patient's visit.
- Ensure lab and imaging results, consultation reports, care transition details, and significant changes in health status are available in the patient's medical record.
- Call patients, consultants, imaging centers, and hospitals to arrange timely delivery of missing information when documents are not present in the medical record.
- Call patients who are deemed to be at risk for a "no-show" before their appointment.
- Coordinate with the BHC or Primary Care Physician (PCP) to schedule "urgent" patients for psychiatry or therapy appointments.
- Assist with training new hires, including one-on-one shadowing.
- Assist with patient complaints when the Behavioral Health Supervisor is unavailable.
- Assist with triaging behavioral health emergencies in the clinic as needed, staff with the appropriately licensed provider, and handle emergencies according to Behavioral Health policies and procedures.
- Attend required mandatory all-staff meetings.
- Provide data entry, general office duties, and administrative support for Behavioral Health at the clinical site.
- Cross-cover remotely and travel to other locations for coverage as needed.
- Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
- Promote effective working relationships and work effectively as part of a team to help the department meet its goals and objectives.
- Attend all required safety training programs and describe responsibilities related to general safety, department/service safety, and specific job-related hazards.
- Attend all required safety education programs.
- Operate assigned equipment and safely perform all procedures as instructed.
- Maintain the work area and equipment in the condition required by Legacy standards.
- Demonstrate proper body mechanics in all functions.
- Check the patient's environment and equipment to ensure safety.
- Report safety concerns promptly to appropriate personnel.
- Follow Legacy's exposure control plans and bloodborne and airborne pathogen precautions.
- Demonstrate knowledge of techniques, procedures, and the correct use of protective barrier equipment, including Universal Precautions.
- Assure a safe environment by instituting appropriate control measures.
- Attend annual education programs.
- Perform other duties as assigned.
Minimum Qualifications
- One year certificate from college or technical school preferred; or one year related experience required
- BLS (CPR & AED) Certified
Required Years of Experience
- One year of Behavioral health or clinical experience required.
Desirable Education and Experience
- Two years of Behavioral health experience preferred.
Required Knowledge, Skills, and Abilities
- Familiarity with Microsoft Word, Outlook, and Excel.
- Good direct and telephone communication skills.
- Must be able to work in a team setting
- Leadership ability for program management, growth, and development within the Center and the Community.
- Communication/interpersonal skills to interact courteously and professionally with associates and customers of various ethnic and socio-economic backgrounds.
- Knowledge of and the ability to interact effectively with various community agencies and resources.
- Coordinate multiple activities simultaneously.
- Computer skills, especially in Microsoft Office (Word, Excel, PowerPoint, etc.).
- Must be able to manage multiple deadlines and decision-making responsibilities.
- Bilingual English and Spanish speaking required
