Occupational Therapist

Occupational Therapist – Full Time We are seeking a full‑time Occupational Therapist to join our in‑house therapy team at Malley Transitional Care in Northglenn, CO. You will work in a collaborative environment serving a diverse patient population. Your responsibilities include creating and implementing therapeutic programs that best serve patient needs. Responsibilities Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care per regulatory and clinical practice standards. Utilize standardized assessments and evidence‑based practice to support clinical interventions. Provide rehabilitative, skilled and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders. Administer procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices. Assess patients’ progress, adjust treatments, and maintain accurate clinical records per regulatory requirements. Evaluate treatment outcomes for effectiveness, modify services accordingly, and make referrals as needed. Provide consultation and counseling to patients, families, caregivers, and other service providers. Communicate with supervisors and interdisciplinary team members regarding patient progress, barriers, and treatment plans. Provide comprehensive discharge summaries of services delivered.
