Description

• Assess and evaluate patients in both inpatient and outpatient environments, crafting personalized treatment plans for their unique recovery needs
• Implement physical therapy interventions that focus on improving mobility, strength, balance, and overall functional abilities
• Work collaboratively with a multidisciplinary team including nurses, physicians, case managers, and rehabilitation specialists to optimize patient outcomes
• Provide education to patients and their families on therapeutic objectives, home exercises, and effective discharge planning
• Maintain detailed documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory requirements
• Engage in interdisciplinary conferences and care team rounds to ensure a seamless continuum of care

Education

• Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited program

Certifications

• Valid state licensure as a Physical Therapist
• Certification in CPR is preferred
