Job Description
Registered Nurse - Case Management

Schedule

12-hour evening shifts, 11 AM to 11:30 PM, with an additional 8-hour shift every other week from 11 AM to 7:30 PM. Overtime is required for additional hours.

Assignment Details

Guaranteed Hours: 36 Weekly
Contract Weeks: 13

Requirements

• 2+ years of Case Management experience required.
• Acute care experience required.
• RN License required; verification must be available.
• BLS certification required.
• Geographical radius rule of 50 miles from permanent address for candidates.
• 5 years of experience may substitute for degree accreditation.

Responsibilities

• Provide case management services for patients within the healthcare facility.
• Collaborate with medical staff to ensure quality care delivery.
• Assist in coordinating patient care across departments.
• Document and manage patient information efficiently.
• Adhere to care standards and safety protocols.
• Be willing to float to other units or facilities as needed.
